Summer Office Hours are Monday-Thursday, 8 a.m.-1 p.m., closed Fridays.
DePaul Prep will resume full-time hours (7:30 a.m.-4 p.m.), Monday - Friday, beginning Monday, August 14.

If your student plans to participate in our athletics program for any season (Fall, Winter, and/or Spring), the following forms must be completed and the following steps must be taken:

  • DePaul Prep must have an up-to-date IHSA Pre-Participation Examination Form (this is in addition to the State of IL Health Exam/Physical form) and Sport Medicine and Acknowledgement Consent Form on file before team tryouts. The IHSA form must be filled out, signed by 1) a physician, 2) your student, and 3) a parent/guardian. You will upload your IHSA Exam Form to your Magnus Health profile. Start Dates: Fall sports begin August 7. Specific team details on tryout dates, times, and additional details can be found here.  Official IHSA sport dates for winter and spring seasons can also be found at the same link.

Fall Sport Registration
Students should register for the fall sports they want to play via the athletics landing page of the website. Fall Sport Registration is open July 3rd through July 31st. Once you have completed this registration, you will receive a confirmation email. Please avoid entering multiple registrations for the same student. Parents of multiple students participating in a fall sport can only register one student per unique email address.

Please note: There will be winter and spring sport registrations prior to the start of those seasons, and we will alert you at the appropriate time.

Pre-Participation Examination/Physical and Consent Form
All student-athletes will need to have a pre-participation examination/physical and consent form on file uploaded to their Magnus Health accounts prior to participating in athletics. Per the IHSA (Illinois High School Association), your athletic physical is valid for 395 days (13 months) from the actual physical date.  To avoid the possibility of a physical expiring and your daughter/son becoming ineligible, we urge you to have the exam during the summer months and before the start of fall tryouts/practices. More detailed information about the preparticipation process can be found on the athletic page of the website. Magnus is now available for all families. Please reach out to if you are unable to login.  If you are participating in a Fall sport, your information must be updated in Magnus before Monday, August 7th.
