Student IDs 

Freshmen and transfer students will take their ID photos on the first day of orientation and will receive a temporary ID to use for the first few weeks of school.

Students will then receive updated IDs with pictures in the following weeks. Students should wear their IDs from the time they arrive at the school entrance doors in the morning until they leave the building for the day.

Dress Code

ACCEPTABLE  SCHOOL ATTIRE

The DePaul College Prep dress code is a major component to prepare students for the professional world while instilling self-respect, neatness and pride in our school identity.  With the support of parents/guardians and faculty, these guidelines help to engender a positive academic climate of mature behavior. Any student who cannot comply with the dress code due to injury or illness must receive written permission from the Dean of Students to attend school out of uniform.  All uniform clothing is to be purchased from the Tommy Hilfiger Website.
 

NOTE: The Administration reserves the right to determine whether a style of dress or a style, length, or coloring in hair is appropriate and in keeping with the philosophy of DePaul College Prep. Teachers are primarily responsible for enforcing the dress code. 

FEMALES 

• White button-down dress shirt, long or short sleeves with school logo, fully buttoned and tucked in.
  -OR- 
  White, navy blue, or lake blue DePaul College Prep polo shirt including dri-fit, clean and tucked in.
   
• Blazer is optional (see Tommy Hilfiger’s website)
• Navy or Khaki docker style pants (chinos) purchased from the DePaul Prep section of the Tommy Hilfiger website.
• Uniform khaki shorts from Tommy Hilfiger are acceptable attire in warm weather or August - September and April - May.
• Yoga-wear, form fitting pants, jeggings, leggings and sweatpants are not allowed; no elastic or tight-fitting cuffs on the ankle. 
• Hair must be neat, clean, and professional.
• Hair color must be a natural color (blonde, brunette, black, auburn) 
• Excessive jewelry is not allowed to be worn in the building during school hours.
• A maximum of two earrings per ear.

MALES

• White, navy blue, or lake blue DePaul College Prep polo shirt, including dri-fit, clean and tucked in.

• Males must be cleanly shaven on a daily basis in school. Sideburns must be neatly groomed and no longer than the middle of the ear. Beards and mustaches are not permitted. 
• One piece of  jewelry such as a ring or necklace is allowed to be worn in the building during school hours. 

ALL Students

• All uniforms must be purchased from the Tommy Hilfiger website.
• Navy DePaul College Prep V-neck, crew neck, quarter zip, or cardigan sweaters are the only acceptable outerwear allowed to be worn in school. A DePaul College Prep polo or shirt must be worn under all outerwear.
  • All outerwear must be official DePaul College Prep apparel from either Tommy Hilfiger, the DePaul Prep Spirit Shop, or team athletic shop. DePaul Prep navy or gray fleece or quarter zip are permitted.
• A school issued DePaul College Prep ID card on the school issued lanyard must be visibly displayed around the neck at all times.
  • Freshman -White lanyard
  • Sophomores - Lake Blue lanyard
  • Juniors - Navy lanyard
  • Seniors - Seniors may wear a lanyard of their choice (must be school appropriate)
• A belt must be worn at all times; the waist of the pants must be above the waist line and pants must extend to the ankles.
• Socks must be worn at all times in school.
• Shoes:
  • Dress shoes
  • Gym shoes  
  • Boots (no combat, cowboy, high heels, etc.) 
  • All footwear must cover the toes and heel (i.e. no open toe shoes or slip-ons)

SPIRIT WEAR FRIDAYS

• DePaul Prep hooded sweatshirts are allowed only on Spirit Wear Fridays. No other brands of hooded sweatshirts are allowed.
• All club and athletic team apparel purchased through DePaul Prep Team Shops is allowed.
• DePaul Prep athletic team jerseys may only be worn on Spirit Wear Fridays
• DePaul Prep apparel from the Spirit Shop is allowed 
• Uniform bottoms and shoes must be worn
• School I.D.s must be worn; uniform shirts must be tucked in 

The following are Unacceptable Dress and Prohibited from being worn in school:

• Piercings (nose and lip rings)
• Hats
• Headgear – including headphones, headsets, earphones or earbuds (except for study hall) 
• Sunglasses
• Visible Tattoos
• Visible chains, medals, medallions

NOTE: The expectation is that students will arrive at school dressed per the dress code and stay in dress code until the end of the school day, unless you are changing for a school sponsored activity. Decisions on dress code matters are at the sole discretion of the Deans of Students. Students who violate this policy may be refused admission to class or asked to call home for the uniform item to be delivered to school.

PHYSICAL EDUCATION UNIFORMS

Students taking Physical Education classes are expected to be in their gym uniform during class.

• PE uniforms can be purchased through our uniform vendor, Tommy Hilfiger.

PE CLASSES FOR THE 2023-24 SCHOOL YEAR

Our plan is to be outdoors and utilize the field turf and green grass across the street beginning on the first day up to the beginning of November. 

Locker rooms are going to be available under the bleachers for students to change when we will be outside. If weather forces us to adapt and have class indoors, students will use any of the four restrooms on the first floor to change for class. Classes will be held in the dining hall or atrium when inside.

9th Grade Requirements

Certificate of Child Health

• Physical Exams completed within 1 year prior to entrance into 9th grade. The exam MUST be completed on the required "Certificate of Child Health" form.
• For 9th graders this form can also be used for the Sports Physical, rather than the  IESA/IHSA form as long as the physician marks "yes" for participation in interscholastic sports. However, the  IESA/IHSA  forms can not be substituted for the  Certificate of Child Health form for 9th graders.
• Parents are required to complete the top portion (medical history) of the Certificate of Child Health form, sign and date it. Failure to do so will result in the form being rejected. 
• Before uploading the physical exam please complete the section titled HEALTH HISTORY. Answer all questions, sign and date.

Immunization Records

• Both the Physical Exam form and immunization record must be submitted by October 15th to be in compliance with the Illinois State Board of Education.

Eye Exams
Eye Exams are only required for students that have transferred from an out-of-state school and this is their 1st year in an Illinois school. This form is due by October 14, 2023.

Dental Exams
Dental Exams are required for 9th graders and must be submitted by May 15th of the current school year and must be completed no more than 18 months prior to the May 15th deadline.

It is CRITICAL that all families enter their child’s medical information into our Magnus system by August 14, 2023. This includes all vaccination information and other required medical documentation. DePaul Prep will strictly enforce childhood immunization requirements and will hold students out of school if these are not in place as is required by the Illinois State Board of Education. Please review the IDPH Immunization Requirements here. See below for how to access Magnus if you have not already done so.

If a student has a serious or chronic health condition that requires medical monitoring, the School Nurse must receive written notification of the condition and treatment from the student’s physician. For students with diabetes, asthma, or seizures, the appropriate action plan must also be obtained from your physician and uploaded to Magnus. The information will remain on file with the nurse@depaulprep.org. 

Visit our Student Health page to download and complete all necessary Student Health forms. All forms will need to be uploaded to your Magnus Health profile. Magnus Health SMR (Student Medical Record), is DePaul Prep's health system to organize and keep all updated immunizations and health-related forms for school and athletic use. DePaul Prep will not be accepting hard copies of health forms; all forms must be uploaded to your student’s Magnus Health account.

INCOMING STUDENTS

Timely submission of health information and other forms is vital so that preparations can be made prior to the first day of school. You will receive weekly reminders as long as there are outstanding items in your account. If there is no data in your account, please reserve 20 to 30 minutes per child. Once information is uploaded, it will be verified by our nurse and if there is anything that needs to be corrected, you will be notified. The good news is that you will not have to repeat this work next year: the data is securely stored, so going forward you will simply provide updates to the record as needed. Anything updated will also be verified and you will be notified if anything needs to be amended.

My MealTime Online

We encourage cashless transactions whenever possible. We are happy to offer you the option to add money to your student’s meal account online through My MealTime www.mymealtime.com. A 4.9% fee is charged by the third party/bank. To avoid the fee, an account balance can be established in advance by check made out to Quest Food Services, and delivered to school. Purchases will be debited against that account balance. 

NEW PARENTS should set up an online account using your student’s “Quest/Meal Time Number” = student ID # (visible after sign in on PowerSchool and begins with a 1). Student IDs can then be scanned to purchase items in the cafeteria. Balances at the end of the year will be transferred to the following school year.

Morning bus transportation is available through the DePaul Prep bus service for an additional fee per semester.

Please Note: Morning bus transportation will not be provided on grade level orientation days.

As we continue to prepare for the upcoming school year, our top priority is the health and safety of our students. The 2023-24 morning bus transportation has been adjusted to accommodate public health and safety guidelines. Please be aware that with these guidelines and restrictions, our buses will not be able to accommodate all families and applicants. The current routes will be altered to pick up students who live furthest from campus. We have identified the following zip codes as areas that we cannot accommodate with our morning bus service:

60607, 60614, 60618, 60630, 60634, 60639, 60641, 60657, 60647, 60625, 60613

Additionally, if your zip code is not listed, a seat for your student is not guaranteed and only 1-2 pick up locations will be on each route at a designated time. Please fill out the bus application by July 17, 2023 if you are interested in utilizing our morning bus transportation. 

You will be notified by August 10 if we are able to accommodate your student and their scheduled pickup time and location.

If your student plans to park on campus for the 2023/2024 school year, you will need to fill out the parking permit form below with their DePaul Prep student email address.

Once the form is completed and submitted, your student will be issued a parking permit. They will be able to obtain these passes on the first day of the school year. Parking tags must be hung from the rearview mirror with the artwork facing out, and displayed at all times. Any vehicle that is not properly registered or does not display a parking tag is subject to being towed. All students with permits may park in either the North or the South lot.

Student Ventra Cards will be available for purchase  at the Reception Desk for $2 on orientation day for Freshmen and Transfers and on the first day of school for returning students. Students are not able to purchase them earlier unless they are enrolled in a Summer School Course.  

Returning students who still have their passes can fill out this form with their DePaul Prep email address during the summer to have their pass reactivated to the student fare on the first day of school

ADDING TRANSIT VALUE TO YOUR CARD
Your Student Ventra Card is active and can be used as soon as you add transit value. You can add transit value online at ventrachicago.com, via the Ventra App (available for download through Google Play or in the Apple App Store), at Ventra Vending Machines at CTA rail stations, at participating local Retailers (use the Find A Retailer map on ventrachicago.com to find a location nearby), or over the phone by calling 877-669-8368.

REGISTERING YOUR CARD
Your card is not automatically registered. Registering your card is optional but has many benefits including protection of your transit value if your Card is lost or stolen, access to convenient account management features online or via the Ventra App, and e-mail notifications with important messages about your account. Register at ventrachicago.com, or by calling 877-669-8368. If you already have a Ventra web account, simply log in and click on Register a Ventra Card. By following this process, you can add new cards to your existing account without having to create a separate online account. It does not matter whether you register the card to the student or parent.

STUDENT VENTRA CARD BENEFITS
This card entitles you to the Student Reduced Fares noted below for trips taken Monday thru Friday between 5:30AM and 8:30PM on CTA buses and trains only. You will automatically be charged the Full Fare outside of these times.

IMPORTANT – RENEWING YOUR STUDENT FARE PRIVILEGES
Returning students who still have their passes can fill out this form with their DePaul Prep email address during the summer to have their pass reactivated to the student fare on the first day of school

HOW TO USE THE CARD
Tap your Student Ventra Card on the Ventra readers to pay for train and bus fare (including transfers) with transit value. Alternatively, to pay with cash/coins on buses, you can request the reduced fare and show the driver your Student Ventra Card. Please note that bus drivers cannot give change and transfers are not available when paying with cash.

REPLACING YOUR CARD IF IT IS LOST, STOLEN OR DAMAGED
You will need to purchase another Student Ventra Card in the DePaul Prep Main Office. Please note that we buy a limited number of cards and may sell out during the school year. If the old card was registered, your parent or guardian can call 877-669-8368 to have a balance transferred to your new card.

All new students are required to obtain a (free!) Chicago Public Library card, or have a library card in verifiable good standing on the first day of school. Information on how to obtain a Chicago Public Library card can be found HERE. E-Cards are not acceptable and an actual physical library card is required. 

School Portrait days will be held on the following dates: 

Freshmen: Tuesday, August 22 during Freshmen Orientation
Sophomores and Juniors: Wednesday, August 23 during Sophomore and Junior Orientations
Seniors: Thursday, September 21 during the school day

Attire: 
Fresh/Soph/Juniors: Students must be dressed in their DePaul Prep polo only. No quarter zips are allowed. If a student does not have the correct polo, one will be provided to them for the picture.

Seniors: Graduation Gown that will be provided.

To view and order photos from Picture Day: You’ll receive an email when your images are ready to view online. Or, you can search for them at vipis.com shortly after photos are taken. You’ll be able to view your images, order packages and select your image for your yearbook (please remember to do so before it’s selected for you), all orders will be delivered to your home.
