Per Archdiocese of Chicago rules, applications will be live on December 1, 2018 at 8 AM.
Following the Catholic School Entrance Exam, prospective students can complete the DePaul Prep admissions application for the 2019-2020 school year.
The step-by-step process will also include uploading the following documents to the application database:
- Birth Certificate
- 8th Grade Report Card
- 7th Grade Report Card
- 7th Grade Standardized Test Scores
- 8th Grade Standardized Test Scores (if applicable)
- Specialized Educational Plan/ IEP / 504 Plan (if applicable)
- Discipline Report
Authorization Form to Release Student Records
Please complete the top section of this form and send to your child's grade school. The grade school will then be able to send your child's discipline report directly to DePaul College Prep via email or mail by the application deadline. Once complete, grade schools should send to: email@example.com

If an 8th grade student seeks extended time or other accommodations on the Entrance Exam, the parent or guardian must submit a copy of their most recent neuropsychological evaluation, as well as an IEP or 504, if one is in place for the student at their current school.
DePaul College is able to support students who need certain accommodations to be successful in a college preparatory curriculum. DePaul Prep provides and extended time up to 50% and small group testing.
